35 by 45 millimetres is the passport and identity-card size derived from the international ICAO standard, used by the UK, the Schengen countries, Australia, Japan, India's PAN card and dozens of others. One size, several head rules
The frame is the same everywhere, but each country sets its own head height within it. The UK asks for 29–34 mm from crown to chin; Australia for 32–36 mm; Schengen visas typically for 32–36 mm (70–80 % of the height). This preset uses the UK figure of 29–34 mm with the head at 31.5 mm, which is inside every one of those ranges. If your country's official page gives a different figure, drag the crown and chin lines until the checklist shows the height you need; the country presets on this site — UK, Australia — carry their own exact figures and sources.
The file
35 mm at 300 dpi is 413 pixels and 45 mm is 531 pixels, so the download is 413 × 531 with the resolution written into the file. That is enough for a sharp print; if a website asks for a minimum pixel size larger than that — the UK's digital route wants 600 × 750 — choose that country's own preset, which outputs the larger size.

What background colour?
It depends on the country: white for many, cream or light grey for the UK, light grey or white for Australia. The preset uses white; the country presets set the correct colour.
Can I use a 35×45 photo for a US application?
No. The US uses 2 × 2 inches with a different head rule. Use the 2×2 preset.
